Default Choking?

In reading through the site, I assume that when a new leaf begins to appear before the previous leaf has fully extended that it is called choking?

If that is the case, then my double mahoi is doing that. Should I assume that it's a function of too much fertilizer along with an excess of water?

Should I rescue the leaf to ensure that it is able to extend properly?
