I've seen some similar holes on other leaves (both Basjoo and Blue Javas), but never before they even open. Is this simply something eating them or a sign of some other problem?

Looks like caterpillar damage. Probably not a problem. Bananas grow too fast to be seriously harmed many one caterpillar. Look for brown liquid\poop. See if it's fresh and if you can find the criminal. It might be in the soil around the plant or maybe under a leaf. I dunno
