After a wicked winter my bananas have bounced back except for my dc mat.
It had the same water and fertilizer as the rest of the banana mats.
Any ideas of what happened?

Well one thing is that there is too many grass and weeds around them. Give them atleast 1ft or 2 ft of space at their base. Its possible that the weeds are choaking the bananas out.

It has some kind of root issue, I bet the roots are rotting. Can you dig up what's left of it and check it out? If not, spray it with a good systemic fungicide.

They are more cold sensitive than others, so may just be finally expressing their displeasure and lack of vigor after last winter. Some plants survive winter, but have a failure to thrive thereafter, and end with a slow decline. If there are pups, you might want to take some off and start over with them.
