Question Root Rot?

Does anyone know what root rot looks like? I have a Musa Basjoo that I transplanted from a small pot, to a bigger one, after I transplanted it, it still was growing, but the leaves started turning yellow, and browning around the edges. also a new leaf tube, came out of the top and didn't roll fully, and started turning black, I cut off a few leaves that were looking bad, and sprayed my plants with a mix of dawn, vinegar, and water. As I was trimming it I noticed another leaf tube just starting to come on of the main stem, so I guess it is still growning. does anyone have any ideas of what could be killing the plant?
