Default Help! Banana leaves are turning yellow (and growth is stunted)

Hey, I'm new to growing bananas. My dwarf Cavendish has recently stopped growing and the old leaves are turning yellow and then shrivelling up. I'm not sure if this is from over or underwatering.

The soil is a mixture of tropical soil, perlite, and a cactus mix. I live in Canada so the plant has always been kept indoors.

Any suggestions would be greatly appreciated!

Update: Here are some pictures of the roots I removed. It looks like fungus to me but I'm not sure. Does anyone know?
