Other people have suggested this, however, the leaves actually are coming out cleanly. Some have reached at least 6-8 inches before suddenly being destroyed.

It looks like it needs some maintenance and trimming. Trim off all the dead stuff so that the new leaves have clearance to get out cleanly. Then you should get good leaves.
