Default dying fruiting stem

so I've had this happen twice now. I have several truly tiny bananas. All are looking healthy, nice green foliage, no spots, no streaks, no insects (haven't even seen spider mites all summer). The first one shot up a flower spike the end of july. i could see the first tiny bananas about 2 weeks later, and then it stalled (it looked like it was sticking in the opening), developed mold and died off. i cut the stalk down. The second equally healthy looking plant popped out a flower spike a month later, and same thing happened. What the heck?? Anyone know what's going on?

So you’re saying it took two weeks to see bananas after the seeing the flag? Weird it’s been a very long time since I’ve grown bananas so I’m rusty but I’ve never had that happen. After the flag shows maybe 1-2 days it goes fast and you see female/bananas immediately sometimes you’ll see 2-3 hands in a single day. But it’s taking two weeks just for one hand?

yes...it's like the stem was stuck in the birth canal or something.

the plant looks green and healthy up to that point. No obvious signs of disease, no rot, no odd spots...
