Wannabe needs help with plant ID
 

The plant below is maybe 4 years old, and has been as big as 5ft or so. It wintered in the house where it got sunlight and seemed to do okay, but just before putting it outside it started to wither and the main plant went away. Out of the root came this little guy, which is doing worse than you see here. I may be watering too much, soil might be wrong, and little fertilizer has been used to date.
Any help appreciated.
I am in zone 6 (Portland, OR), in case that is important.
