I am not an expert, but I have some experience with bananas, and a lot with gardening in general.
It looks like it has only happened once to the first leaf... am I right?
My first thought was possibly a squirrel jumping down from a nearby tree onto the leaf, or possibly climbing up it. Squirrels in the midwest are much bigger than these puny tree rats in Florida.
I hope that helps or at least holds you over until one of the resident experts here sees your post. Make sure the center is clear so it can grow and wait and see if it happens again to the next leaf.

Sometimes mine will be weaker than normal after a lot of rain and fertilizer. Looks like one leaf is curing towards the end. That's usually a sign of too much fertilizer biased on my experience. Could be squirrels or wind or anything. Wouldn't take much if the leafs are weak. Either way they should correct themselves with the next couple leaves.

A lot of time and experience; the same way I can look at a cat's face and tell if it's male or female. Each variety has its own unique characteristics that must be learned over time, until they become familiar enough to spot.
