I have a bed of banana plants growing in my garden but they do not last very long. What should I do to save the plants?

what medium are they growing in and are you fertilizing too little/much? pics would really help find out what's going on.

I do not have a picture of the banana plant. However I can explain that when I cut the plant, I found that the center of the stem was rotting. Is this due to too much water in the soil or other causes. This problem has been there for sometime with all the new shoots that come up. The grow well at the beginning but when they are about 3-5 feet tall the leaves start drying up and the plant die. I did not have this problem earlier as I was harvesting good bananas.

Maybe as you say the soil is wasted away. Therefore I will need to dig up the whole area, remove all the old roots and replant with new baby plants. Thank you

As you can see from my location I live in Malaysia, which is in the tropics and it is always hot and hot all the year around. Thank you
