if you have a corm with 2 pups such as:
1 A 2
where A is the mother plant, once it fruits and dies off, will that particular section of corm ever reproduce another pup? or will it just continue to expand outward?

Your situation may be different - I assume you are trying to maximize pups. In the tropics the issue is the opposite - too many pups and then the bunches and fruits tend to get smaller and smaller each year. By the time of the first harvest, we tend to have at least 5 healthy pups fighting for the space that had originally been ideal for just the 1 that was planted.
To get to your question ... the assumption/prejudice is that the old mother corm is part of the problem, that it will continue to pump out the pups. (I never have, but I bet others have - you could probably plant the mother corm in some other location where you wanted more plants and see what happens. I would be interested.)

not trying to maximize pups or anything, just a general question, whether or not the mother corm would eventually regrow, or if that spot was a dead spot

If it becomes a large corm I've had something come up in the same spot multiple times.

Does the P-stems also get smaller or just the bunches and fruit?
I may be having this problem. I have a large fruiting P-stem, a couple P-stems2/3 the size of the main and then 5-7 pups up to five foot tall. My recent bloom only had two hands and a total of twelve bananas.
Do I have too many pups?

I think you have too many pups.Keep only 2 or 3 pups or no pups untle its fruits.
