I'll put my two cents in:

I mix up my own and it comes out to approximately 11-4-37. Here is my recipe:

Urea (46-0-0) 1 cup
Triple Super Phosphate (0-46-0) 1/3 cup
Muriate of Potash (0-0-62) 2.5 cups
Essential Minor Elements 1/4 cup

I use 1 pound (two cups) per plant per month during the growing season. This equals approximately a pound of nitrogen per plant per year. For bananas that get a bit fussy if you give them too much nitrogen at once (like my Raja Puri), I use one cup every two weeks. I buy the urea and muriate of potash in 50 pound bags from a local lawn supply store. The triple super phosphate and the minor elements are available on Amazon. All of my bananas seem to like this mixture just fine.
